For written consent for surgery the DH (2002) have
produced four versions of the standard consent form
Form 1

For competent adults. Legally competent children
and young people (16-17 years) consenting to treatment may also
sign the 'adult' form 1, but may like a parent to countersign as
well.

Form 2
|
For parental consent for a child or young person,
although the child may also sign this form.

Form 3
|
For cases where it is envisaged that the patient
will remain alert throughout the procedure and no anaesthetist will
be involved in their care.

Form 4
|
For adult patients unable to consent to
investigations and treatment.
