Due to the increased diameter of wide bore devices conventional low pressure ventilation is effective. This includes all forms of bag and mask ventilation, all conventional ventilators, all anaesthetic fresh gas sources and even gas delivered from an anaesthetic machine flush.
Expiration can take place, either through the device, or through the airway if it is patent.
If a cuffed device is used, conventional, i.e. low pressure source, ventilation is used and expiration takes place through the device.
If an uncuffed device is used (not recommended), it may be necessary to obstruct the upper airway during inspiration to prevent loss of all ventilating gas via the upper airway.
