The lateral recovery
position is generally the safest position for patients following
surgery. In this position, vomitus and other secretions are less at risk
of being inhaled. The tongue falls forward and is less likely to obstruct
the airway in this position. The left lateral position is usually
recommended, because laryngoscopy and re-intubation are easier on this
side when using a standard Macintosh blade.
The following patient positions may also be considered at
extubation:
- The supine position is
acceptable if the patient is awake with good airway patency and there
is little risk of regurgitation
- Sitting the patient
up at extubation may also be acceptable. Lung ventilation is
usually better when sat up, so this position may help patients with
COPD (Chronic Obstructive Pulmonary Disease) and other respiratory
disorders. Obese patients and those with cardiac disease often breathe
more comfortably when sitting too. The sitting up position is also
useful after certain head and neck procedures, as it facilitates venous
drainage, and often provides a more patent airway. Hypotension is a
contraindication
