A number of other measurements may also be recorded (Fig 1).

The conscious level should be assessed in all obstetric patients and recorded in the Neuro Response section, using the AVPU scale:

• A - Alert and conscious
• V - Responds to voice
• P - Responds to pain
• U - Unresponsive

'S' or sleeping is not an acceptable recording because it may miss a patient who is unconscious, e.g. from shock.

Pain is scored as follows:

• 0 - No pain
• 1 - Mild pain
• 2 - Moderate pain
• 3 - Severe pain
