The total number of yellow and red scores should be entered for each hour and action taken as soon as the total score reaches the following:

Yellow	Red	Action that must be taken
1		Continue observations
2		Medical review required
	1	Medical review required, anaesthetist should also be called

After a triggering episode, frequency of observations should be increased to every 15 minutes.
