You should recommend a neuraxial technique if you believe there are clear benefits to the patient. There will be times when a neuraxial technique is not indicated. Sometimes there may be no preferable technique. If you believe the neuraxial technique and alternative are equally good, say so. In this event, patient preference becomes an even more important determinant.
Beware misapprehensions and biasses on the part of the patient, surgeon and indeed anaesthetist, which can influence the decision-making process adversely.
A clear understanding of the issues will enable you to have an enlightened discussion with your patient, and will ensure you both reach the right decision.
