Question: How do you measure pain?

Answer: To measure pain:

• Ask the patient if they have pain
• Ask them where the pain is
• Ask if it has got worse
• Document a pain score 0 - 10, where 0 = no pain and 10 = worst pain

Pain may represent a developing problem.
